I am currently working on our 501c3 Application and I have most of it completed; however I have gotten stuck at several points and was interested in using your sample kit for either a church or outreach organization:

I have several questions prior to making such purchase:
1. Our doctrince and some of our initiatives resemble that of a church and outreach ministry even per your sample kit, I keep getting stuck on should we file as a church or public charity.
* In our articles of incorporation (which have already been approved) states that we are organized for religious purposes.
2. Do your sample kit include the actual 1023 application according to the relative sample kit (i.e, do you include a completed 1023 for church, a completed 1023 for an outreach organization)
3. I have viewed your website, could you list the documents you get for the $99.00</description>
